Key Milestones Explained
Dating / Viability Scan (7-12 weeks)
An early ultrasound can confirm pregnancy, check for a heartbeat, and establish or confirm your due date. ACOG recommends at least one standard ultrasound during pregnancy, typically at 18-22 weeks.
First-Trimester Screening (11-13 weeks)
The nuchal translucency (NT) scan measures fluid at the back of the baby's neck. Combined with blood tests, it assesses the risk of certain chromosomal conditions.
Embryo → Fetus Transition (Week 11)
ACOG states that the embryo is called a fetus from week 11 onward. By this point, all major organs have formed.
Quickening (16-22 weeks)
Quickening is the first fetal movement the mother feels. In first pregnancies, this typically occurs at 18-20 weeks. In subsequent pregnancies, it may be felt earlier (14-18 weeks) because the mother recognizes the sensation.
Anatomy Scan (18-22 weeks)
The anatomy scan is a detailed ultrasound that checks the baby's development, including the brain, heart, spine, and other organs. ACOG recommends at least one standard ultrasound at 18-22 weeks.
Periviable Period (20 0/7 – 25 6/7 weeks)
The periviable period is the range of gestational ages where survival outside the womb is possible but not guaranteed. ACOG/SMFM Obstetric Care Consensus #6 provides survival statistics:
| Gestational Age | Survival Rate | Significant Morbidity |
|---|---|---|
| Before 23 weeks | 5-6% | 98-100% among survivors |
| 23 weeks | 23-27% | High |
| 24 weeks | 42-59% | High |
| 25 weeks | 67-76% | Moderate |

Third Trimester Begins (28 0/7 weeks)
The third trimester begins at 28 0/7 weeks. This is when the baby begins rapid growth and the mother may experience new symptoms like shortness of breath and frequent urination.
Term (37 0/7 weeks)
ACOG classifies 37 0/7 weeks as "early term." Full term is 39 0/7 – 40 6/7 weeks. Late term is 41 0/7 – 41 6/7 weeks. Postterm is 42 0/7 weeks and beyond.
Estimated Due Date (40 0/7 weeks)
The due date is an estimate. Only about 5% of babies are born on their exact due date. Most are born within 2 weeks before or after.
